This form is for our 2010-2011 fall session. It is ONLY a request form, NOT the registration form which needs to be filled out and returned with payment.  Please read all information below to assure that your request is accepted.  Also note, below the class selections on this form, the age requirements for determining which class is appropriate for your student.  Thank you!

 

Registration Instructions - PLEASE READ! 

  1. To begin the registration process, you must first complete a Request Form. Request the class(es) that you want to register your child for. You may indicate several classes that you wish to register for, and you may also indicate an alternate choice if your 1st choice of class is unavailable. You must enter all required information for your request to be sent. It is necessary that we have your current email address so that you can be contacted about your child’s registration.
  2. Once you submit your request, you will receive a message stating that your request was sent. If you do not see this message, your request did not go through and another one should be sent. 
  3. Within ONE WEEK of your request, you will be sent an email with confirmation of your child’s class registration. Please note: You will receive one email per registered class. Therefore, if you register for more than one class or more than one student, you will receive more than one email confirmation. These confirmation emails may (or may not) be sent at the same time.
  4. Follow the instructions that are sent within your confirmation email. These instructions will ask that you print a registration form from our website. This registration form must be filled out and mailed to the studio. The required registration fee and at least first month’s tuition must accompany the registration form. Because our office hours are limited during the summer, the registration form and payments must be mailed. The registration form, registration fee, and tuition must ALL be received within one week of your email confirmation to secure your child’s spot.

TO ENSURE THAT EACH MEMBER IS REGISTERING FOR THE PROPER, REQUIRED CLASSES, 

SEE THE PERFORMANCE COMPANY PAGE TO VIEW EACH GROUP'S REQUIREMENTS.

NOTE:  You may choose an older age category, if your child will be that age by November 1st.  For example, choosing the 9-10 Jazz class is allowed, if your child is now eight years old, but will turn nine years of age before November 1st.
